Both thyroid dysfunction and heart failure show a high prevalence in general adult population; frequently in clinical practice, a multidisciplinary approach is useful to optimize the patient’s management. Although there is no doubt that a close link between cardiovascular pathophysiology and thyroid homeostasis does exist, our knowledge on the field is, however, far to be exhaustive. Thyroid hormone regulates the expression of cardiac specific functional contractile and structural proteins and plays a pivotal role in modulating both diastolic and systolic function as well as peripheral vascular resistance. The strict relationships between thyroid and heart dysfunction is strongly supported by recent evidences on a key role of an altered thyroid profile as negative prognostic predictor namely in patients with heart failure. The treatment for chronic heart failure expecially in advanced stages is an open and challenged field. The potential for novel thyroid hormone therapies also based on molecular biology may thus represents an actractive area of multidisciplinary scientific interest.
