This book is a collection of chapters written for the 2006 meeting of ''Controversies and Up-dates in Vascular Surgery'' held in Paris. In the book, you will find the most up to date information required for current daily practice as vascular surgeons. However, not all decisions in surgery are evidence based as many patients fall outside the convenient frame of study groups observed in randomised studies. Among the wide range of technological advances designed to overcome technical problems or specific management issues, many will not stand up to the robust scrutiny of clinical evaluation. However, the few which prove to be efficient will help improve the care of patients and the understanding of their diseases. INDICEThe fate of the endograft after 5 or more years1/3/6/12/18 months and life long yearly assessment is the routine: is it right and safe for patients with EVAR?The plain abdominal radiograph is a useful technique for surveillance after EVARBranched endografts for abdominal aortic aneurysms with bilateral common iliac aneurysms extending to the iliac bifurcationShort neck aneurysms: do we really need fenestrated grafts? Of course and I will prove my caseAre fenestrated grafts useless, time-consuming and costly? The outcome of EVAR in AAA with a short infrarenal neck in the EUROSTAR registryCan we predict aneurysm rupture?Gender matters in abdominal aortic aneurysmsEVAR is the best option at one month but is it still the case at longer follow-up?More than 1000 stent-grafts with 10 years follow-up and only 6 ruptures: what is our policy to prevent catastrophic events? In Perugia 40% of necks enlarge and cause problemsIn Sidney no neck enlargementShould type II endoleaks be treated after endovascular abdominal aortic aneurysm repair?Intrasac pressure measurements in the presence of type II endoleaks before and after endovascular treatment of AAAsLate failed EVAR: when to choose surgical conversion or endoconversionAneurysm sac occlusion using OnyxExpansion of the aneurysm after endovascular repairTreatment algorithms for ruptured abdominal aortic aneurysmsManagement strategies, adjuncts and technical tips to facilitate endovascular treatment of ruptured abdominal aortic aneurysmsAbdominal compartment syndrome following ruptured AAACarotid stenosis: do we operate the the right patients? The timing of carotid endartectomy after strokeIs it ethical to perform carotid artery stenting in asymptomatic patients?Factors which may induce restenosis following carotid surgeryEvidence for the routine use of carotid filters during carotid artery stentingWhat to do when a stroke occurs during carotid artery stenting?Does carotid artery stenting work on the long run: 5-year results in high-volume centers (ELOCAS registry)Do carotid stents fracture?EVAR state of the art 2006Effect of graft planarity on outcome after infra-inguinal reconstruction and development of a novel helical graftWhen and how to prescribe antithrombotic therapy after lower limb revascularisationAspirin and Clopidogrel may improve infra popliteal bypass patency: rationale for a new trialPeripheral graft infection: cryopreserved arterial allograft reconstruction is the best optionSubintimal angioplasty: how I do it and what I do if complications occurCutting balloon for leg arteries, the ultimate tool?Do we have the right stents for femoro-popliteal lesions?Endovascular repair of popliteal aneurysmsExperience with endovascular repair of popliteal aneurysmsPopliteal aneurysms: surgery offers durable resultsEmbolic lesions from the aortic arch during aortic stent-graftingCurrent limitations of thoracic stent-graftsHybrid procedures allow endovascular treatment of aneurysms and dissections involving the aortic archProspective randomized trial: private practice is not a limitation to participationStent-grafts are now reimbursed: is this going to change our practice?Complex bypasses and fistulas for difficult hemodialysis access – a prospective series of 24 consecutive cases from a single centreFifteen years ago laser was supposed to open the arteries, now it is supposed to close the vein: what is the reality behind the tool Endovenous laser ablation of the small saphenous vein: how to avoid neurologic complicationsValvuloplasty of incompetent deep venous valvesMinimally invasive retroperitoneal approach for the treatment of abdominal aneurysmTotal laparoscopic repair is my routine practice: my latest tricksCan all patients requiring aortic repair be treated laparoscopically?Laparoscopic aortic surgery: recent development in instrumentationTotal laparoscopic AAA and iliac artery aneurysm repair using hybrid techniquesLaparoscopic occlusion of lumbar arteries during AAA repairABSTRACTS OF SYMPOSIUM: AMBULATORY TECHNIQUES FOR VARICOSE VEINSLong term ultrasound findings at the sapheno-femoral junction after radiofrequency endovenous obliteration of the greater saphenous vein: preliminary study Ambulatory surgery: stripping, EVLT, closure: what to choose?Sclerotherapy and thrombophilia.
