The aim of this book is to share the experience built up over a 40-year career dedicated to periodontal therapy, and it is underpinned by a few fundamental criteria: a correct diagnosis, an effective treatment plan, its rigorous execution, and an appropriate follow-up.
For therapy to be successful, the operator must be aware that each patient represents an individual case. Biologic and scientific training as well as clinical and surgical skills must be gradually backed by clinical experience and the ability to establish a “therapeutic alliance“ with the patient. This latter factor is essential when dealing with long and complex cases: the patient’s expectations and the commitment of the clinician must work together harmoniously.This book attributes particular importance to the long-term assessment of the results obtained. Success achieved immediately or during the very early years after treatment is of little relevance and interest.
Patients expect an expert clinician to provide a prognostic assessment and an outcome that will last at least 10 years. This applies to all types of treatment, from those that are exclusively periodontal to multidisciplinary treatments requiring combined periodontal, endodontic, orthodontic, prosthetic, and implant therapies. Only on these premises is it possible to help patients select the therapeutic option best suited to their needs and expectations, first and foremost that of saving their own natural dentition.
This last choice often involves more difficult, more demanding, and less remunerative forms of treatment than implant therapy alone. In this book, I have chosen to deal specifically with periodontal therapy, without considering implant therapy (though this remains an indispensable treatment option in partially or fully edentulous patients) in the conviction that only a knowledge of the long-term behavior of bone and gingival tissue can guarantee the success of treatment, including implant treatment, where soft and hard tissue stability is the key to long-term success.
In writing this book, I have also tried to distinguish between procedures backed by scientific evidence and those that are performed on the basis of clinical success achieved over time by the various authors, in the awareness that many routinely performed therapies are not supported by scientific evidence even though they guarantee effective results.
