Myocardial ischemia, far decades, has been perceived as closely linked to coronary atherosclerotic lesions, and this (mis)conception has dictated the diagnostic and therapeutic strategies: invasive or non-invasive visualization of atherosclerotic lesions has been accepted as a surrogate far evidence of myocardial ischemia and removal of these obstructions was expected to cure this condition.
In recent years, an overwhelming body of evidence has strongly challenged this "steno-centric" hypothesis, and the complex, multifactorial, and dynamic nature of myocardial ischemia has progressively emerged. Eventually, in the 2019 update of the ESC Guidelines, a major nomenclature shift has been proposed substituting the word "disease" with the word "syndrome:" a clear acknowledgment of the multifactorial nature of this condition.
This new understanding of myocardial ischemia was expected to promote major changes in the approach to diagnosis, therapy, and prevention. Unfortunately, an unbiased look to current cardiology practice shows a surprising inertia in transferring this cultura! evolution into diagnostic and therapeutic protocol.
The purpose of this book is to promote a clinica! management of myocardial ischemie syndromes consistent with the new understanding of this condition in arder to improve quality of life and prognosis of patients. Expert cardiologists, that have personally contributed to this cultura! (r)evolution have contributed to this volume focusing on the diagnostic and therapeutic implications of the shift from disease to syndrome.
